Welcome to the Patient Portal Account Request page.

Please allow up to 48 hours for a response. Responses are handled Monday – Friday in the order they are received in the system.  (Saturdays and Sunday do not count as part of the 48 hours)

Please do not send multiple request as this will cause a delay and can cause issues in setting up your portal account.

For security reason we can only activate portal accounts or send information regarding portal accounts using the email address we have on file.

If you need to update your email account contact your Medical Staff.

    *Please note that two patients can not use the same email address to two separate accounts.
